Dear Sir, My name is AVIJIT SINGHA DL NO. WB-4120050153819 Issue date-23.10.2013 Print Date 03.12.2013 from Burdwan Licence Authority, I have received my Renewal Driving Licence on 06.12.2013 but my Father name, Date of Birth & Blood Group wrong printed. We have provide earlier all document from our end please arrange to issue the correct Driving Licence, so kindly see the matter and take the necessary action.
